All of our appointments, including urgent on-the-day appointments are now requested through our online form system. There are several ways to access this form.
- NHS App – Login to your NHS App, click ‘Services’, click ‘Contact your GP about a health problem’.
- This website – Click this link to complete an online form – Contact us about your request.
- If you cannot access a form online, please contact the surgery to speak to one of us who will be able to help with your query.
Housebound Patients:
If you are confined to your home through frailty, illness or after an operation and need health advice/help, then please contact us to discuss a home visit. You can do this by submitting an online form as above. A friend/relative can complete this form for you.
Preferably let us know if you require a home visit before 10:30am.

Doctors Appointments:
We have a mixture of urgent on-the-day appointments and pre-bookable appointments. We are offering both face to face and telephone consultations.
Nurses Appointments:
Our Nurses are trained in the following areas:
Chronic Disease management, immunisations, well woman clinics (including contraception/ cervical smear tests/coil removal/swabs for infections/HRT), over 75 health checks, travel vaccinations, dressings.
Some conditions are managed by individual nurses to high level of expertise.
Please state what type of appointment you want when booking – some things are being done via telephone consultation.
Health Care Assistant Appointments:
Our Health Care Assistants support the Practice Nurses. Carrying out tasks/procedures such as:
Blood tests, blood pressure management, B12 injections, initial diabetes checks and more.
Please state what you are attending for when you book an appointment.

Cancelling Appointments:
Please help us to help you. If for some reason you need to change or cancel your appointment, you can do this via the text reminder sent to your phone. If you unable to do this, you can cancel appoinments through the NHS app or please telephone the surgery as soon as possible.
When appointments are not kept, it prevents another patient from being seen. We routinely write letters to patients who do not attend or cancel their appointments.
